Please download and complete the necessary contracts for the service(s) you have booked, and a Veterinary Release Form for each pet. Once completed, kindly sign and return the contracts and Veterinary Release Form(s) either by mail to Walk or Sit My Pet at 3 Farm Ground Close, Hook, Hampshire RG27 9SE or by email to sharon@walkorsitmypet.com before the start date of your chosen service(s).

Please note that by completing and returning the appropriate documents duly signed, you confirm that you have fully read and understand Walk or Sit My Pet’s Terms and Conditions.
